Dette innlegget er sterkt preget av egne og andres erfaringer.
Vi får høre at man må be om hjelp for våre psykiske vansker, men samtidig opplever alt for mange pasienter at det ikke hjelper å be om hjelp! Noe må gjøres hurtigst mulig for vi har ingen liv å miste. Jeg har ved flere anledningen fått førstehåndserfaring fra ulike brukere, venner og bekjente. I dette innlegget forsøker jeg å beskrive et utvalg av de mest alvorlige problemene med dagens psykiatri og komme med noen tanker om hvordan dette kan bedres.
Hvorfor ber ikke folk om hjelp? Mye har nok med å gjøre at man gang på gang møtes med noe liknende «da du har AD/HD og asperger er det helt umulig å utrede deg for bipolar lidelse» eller at «vi tror ikke du er suicidal, det er bare AD/HD».
Alle verdens problemer kommer ikke av AD/HD!
Det er forholdsvis store forskjeller på bipolaritet, AD/HD og autisme til at diagnosene kan stilles gjennom grundig utredning. Problemet er bare at alt for mange blir møtt på samme måten som meg hvor det ikke blir utredet noe som helst. Jeg møtte opp på DPS to ganger, men alt etter første hadde hun gitt opp. Det føles som de aldri prøver…
Vi trenger en ny psykiatri hvor pasienten er i sentrum og behandlingen ikke utelukkende skjer på institusjon, men heller gi pasienter muligheten til lengere opphold i naturnære forhold med dyr inn i behandlingen.
Samtidig trenger vi også den mer konvensjonelle psykiatrien, poenger et bare at ikke alle er like og vi må derfor ha helsetilbud som et pasientorientert, ikke systemorientert.
Men hva kan vi egentlig gjøre for å bedre psykisk helsevern? For det første hadde det hjulpet om politikerne hadde økt bevilgningene, for det andre trengs det flere døgnplasser og flere lavtelskertilbud. Videre er jeg overbevist om at behandling utenfor sykehus, som f.eks. lengre opphold på småbruk med dyr, natur og få stressorer ville kunne ha en svært positiv effekt på mange.
Alt kan dog ikke fikses med økte bevilgninger og alternative tilbud, for vi trenger også kompetente behandlere, noe det i dag er mangel på om du spør meg. Det handler ikke om at de ikke har utdanning, men heller at de mangler evnen til å se tingene fra pasientens perspektiv. Gang på gang opplever jeg selv eller hører om andre som opplever at behandleren(e) ikke klarer å stille spesifikke diagnoser med begrunnelsen at det er for komplisert. Selv ble jeg henvist til utredning for bipolar lidelse type 2, men siden jeg har AD/HD og asperger mente psykiateren jeg hadde på DPS at diagnostisering av bipolar ble for vanskelig.
Det finnes mange andre eksempler, f.eks. om folk som blir kasteballer i systemet hvor den ene poliklinikken skylder på AD/HD og vidrehenviser, og AD/HD poliklinikken skylder på PTSD og stemmehøring og vidrehenviser, og den runden går i en evig sirkel.
Det overrasker meg ikke at folk begår selvmord .
Jeg forstår godt at folk begår selvmord , for når en gang på gang ber om hjelp, men blir avvist hver gang, eller behandlingen ikke har effekt mister selv den mest optimistiske person håpet om bedring.
Ofte opplever jeg at det er vrangvilje som forårsaker de største problemene. Et eksempel på det er når behandleren nekter å godta at en har språkforståelsesproblemer og ignorerer pasientens behov for tydelig kommunikasjon.
Det er ingen tvil om at psykiatrien i Norge er underprioritert, noe som kan forklares med at psykiske plager bare er i hodet og ikke synlig for andre. Hadde det vært kreft ville man få behandling, men om man er høy (hypoman/manisk), men samtidig deprimert og suicidal er det ikke på langt nær sikkert at en får noe hjelp overhodet.
Er ikke suicidalitet like alvorlig som somatisk sykdom?
Det er omlag 12% av den generelle befolkningen som begår eller forsøker å begå selvmord . De tilsvarende tallene for bipolare i depressiv fase er 25% og ved autisme 36% , noe som er alarmerende høyt og virkelig viser at disse pasientgruppene ikke opplever å motta et adekvat tilbud.

En annen ting det for meg virker som at psykiatrien mangler er forståelsen av at de fleste psykiske tilstander faktisk er biologisk betinget. Ved depresjoner kan kognitiv atferdsterapi være meget effektivt, men medisiner som ketamin og fluoxetin kan være et godt supplement, spesielt ved alvorlig depresjon.
Problemet er slik jeg har erfart det er dog ikke størst ved lavt stemningsleie med depresjon, for da får en lettere hjelp, men ved høyt energinivå og maniske tendenser med depresjon blir en ofte møtt med at “du er ikke deprimert, men hypoman/ manisk”. Av egen erfaring kan jeg skrive under på at det er fult mulig å ha energinivået, manglende fokus, tankekjør osv. til manien og samtidig være deprimert og på grensen til suicidal.
Selv om det er MYE mer som både bør og kan nevnes velger jeg nå å begrense meg til en siste ting. Det er på ingen måte greit å få beskjed om “at du klarte å studere og er så smart at du kan klare å gå på nettet å fikse problemene dine selv”. Jeg er generelt vanskelig å målbinde, men når jeg fikk høre at personen jeg snakket med hadde fått den beskjeden mens hen var innlagt må jeg innrømme at selv jeg ikke klarte å finne ord.
“Pasient fikk akuttime. Først etter åtte uker skulle han kalles inn igjen, men behandler fant ham ikke i systemet. En grundigere sjekk viste at pasienten hadde tatt sitt eget liv.”
Selvmord i psykiatrien
Kampen for å overleve er ikke lett, spesielt om en har hatt en turbulent start på livet. Når vi da også ser en høyere forekomst av psykiske lidelser i den generelle befolkningen er det mildt sagt kritikkverdig at vi i dag har omlag 8.554 færre sengeposter i psykiatrien en i 1980. Med andre ord hadde vi i 1980 omlag 12500 sengeposter, mens vi per 2017 bare hadde omlag 3746 sengeposter.
Forsømmelsen av psykiatrien har en stor kostnad for både enkeltmennesker, pårørende og kan i lengden på ingen måte være samfunnsøkonomisk ulønnsomt. Dessverre er det ofte de svakeste i samfunnet, barn, utviklingshemmese, funksjonshemmede, schizofrene osv. som rammes hardest av slike nedskjæringer. Og få om noen med psykiske vansker tjener på denne utviklingen .

Ikke bare er det voksenpsykiatrien (VOP ) som svikter, men også barne- og ungdomspsykiatrien (BUP ) klarer heller ikke å levere de tjenestene/ den behandlingen pasienten har rett på .
For å si det rett ut er psykiatrien i dag lek med folks liv og helse.
